Carer's Assessments

Adult Carers looking after another adult over the age of 18

Sunderland Carers' Centre delivers carer's assessments on behalf of Sunderland City Council for adult carers looking after another adult over the age of 18.

What is a carer’s assessment?

A carer’s assessment is your opportunity to discuss how being a carer affects your life. You can discuss any changes you might like to make and how you can be supported to achieve these.

A carer’s assessment is not an assessment of your capabilities as a carer, though importantly you will be asked if you are willing and able to continue to provide care.

The assessment could be face to face, between you and an assessor, a supported self-assessment online or a phone assessment. We have attached a copy of the assessment form to this page, so that you can see what questions you will be asked. If you would prefer, you can also complete the form yourself and send it back to us.

What will the carer’s assessment cover?

The carer’s assessment will look at the care and support you provide, as well as providing information and advice to support you in your caring role. The assessment will also look at anyone else who may be affected by the caring role. This will include identifying any young carers and assessing what support may be available to them.

If the person you look after is also being assessed, you can be assessed at the same time (as long as you both agree to this).

You can also have a carer’s assessment even if the person you look after does not receive services or refuses services.

What happens after a carer’s assessment?

We will give you a copy of your assessment and a support plan. This will provide details of the outcomes you have identified and how they are going to be met.

This could include giving you a personal budget if you meet the eligibility criteria.

If you would like a carer’s assessment or more information about the carer’s assessment, please contact Sunderland Carers’ Centre on (0191) 549 3768.

Parent Carers looking after a child or young person under the age of 18

Parent Carers looking after a child or young person under the age of 18 can ask for a carer's assessment under the Children and Families Act 2014. This is different from the Care Act 2014 which only applies to those who are providing care to adults.

If any needs are identified during an assessment of a Parent Carer there is no duty to offer a personal budget to meet these needs. However, there may be other information and services to support Parent Carers to meet their needs.

Young Carers

If a Young Carer is caring for an adult they may be assessed under the Care Act.

Please contact Sunderland Carers’ Centre on (0191) 549 3768 for information about the services and support available to Parent Carers and Young Carers.
